Required Documentation

• Breed Survey or Resurvey
  • Needs registration papers (SV, AKC, or other)
  • certified four generation pedigree
  • Hip certification (OFA or “a” stamp)
  • USA certified scorebook (must include AD)
  • Conformation rating (conformation rating from 24+ without title does not count)
  • USA registration or registration pending letter
  • current USA membership card
  • If resurvey, you must include original survey.
  
  
• Puppy Classes 4-6, 6-9, 9-12 months
  • current USA membership card (required)
  • registration papers (SV, AKC, or other)(not required but preferred)
  • certified four generation pedigree (not required but preferred)
  
  
• Youth Dogs 12-18 months and Young Dogs 18-24 months and Adult 24+ months untitled

Some Reminders:

1. If you do not have USA/SV papers for your dog, please contact the AKC at 919-233-9767 to obtain a certified pedigree. A pedigree from a breeder or pedigree service will not be accepted.
2. All scorebooks must be USA certified or a $50 bond will be required.
3. As of July 1, 1995, ALL DOGS 12 months and older, shown in a regular conformation class, must be USA registered or the registration must be pending. If your dog’s registration is pending, please obtain a fax or letter from the United Schutzhund Clubs of America. For further information, please contact the USA office at 314-638-9686.
4. Dogs over 3½ years of age must be surveyed to be eligible for a “V” rating.
5. Entries are non-refundable and no entries will be taken after closing date.
